Create a Form in Word

 Turn on the Forms toolbar  -- Click View/Toolbars/Forms

 

Text Fields

Create your document like you normally would except click   everywhere you want to enter a text field in your form.

To make a field shaded or unshaded click this button . I have all the fields shaded in this document.

example:

Fill in the following information:              Name:                    Date:     

 

When you have your document completed, click the lock button   to protect the form and activate any drop-down boxes you have created. It is a toggle button, so click it again if you wish to unlock the form. (You can also use Protect Document to prevent changes to other text. (Tools menu, choose Protect Document or Tools/Unprotect Document)

When someone fills in the form, they will only be able to type in information in the form fields.

Form Field Options

You can limit the number of characters that can be entered in a field by clicking on the Form Field Options button.

You will get the following window. I set the length to 20 in the example. You can also change the Type to Date, Numbers, and more.

 

Other Forms buttons

Click the checkbox button to insert a checkbox field.     After clicking the drop-down form field button, double click the gray box that appears in your document to add the items to your list.

You can also add a table and a frame from the Forms toolbar.

You will still have to enter a text field in here for someone to be able to type.
